Who We Are

The Health Leadership Network (HLN) was launched in 2002 to develop a venue for public and private entities to respond to priority health issues in Lake County, California, and to work collaboratively to better population health countywide. Over the past 15 years, the HLN has engaged many local, state and national organizations and agencies, paving the way for numerous cross-sector collaborations, and continues serving as a catalyst and pioneer; seeding systems, environment and policy change that reflect community and partner-driven priorities and align efforts aimed at promoting and sustaining a cultural shift toward wellness. 

The HLN’s vision “Partnerships for a Healthy, Thriving Lake County,” actively calls upon the expertise of representatives from local county, education, healthcare and community based organizations to achieve its mission of “providing innovative leadership to build system connections among service agencies and healthcare providers that identifies best practices and seeds change.”
